When you access our community nursing care services, you will receive a single point of contact case manager that you can rely on. Promoting independence is ideally the goal of disability services, however we understand that this isn’t always an achievable reality.

We come to you, evidence-based wound management practice – contemporary healing products and techniques,
NDIS will fund a registered nurse to provide wound management if it is caused by or directly related to your disability, including dressing changes and care for chronic or acute wounds, pressure sores, or post-surgical wounds.
Clinical Nurse Consultant services including Continence Assessment and Report + Assistive Technology Recommendation. and private enterprise).
Blood glucose monitoring, insulin injection, sliding scale dosing, and diabetes education, especially for people unable to self-manage. This service includes overarching case-management and care planning from a clinical nurse consultant.
Support with bowel management, including suppositories, or enema administration for individuals with complex bowel care needs. Complex bowel care plans and Disability Related Health Supports report.
Assistance with seizure management, including the administration of emergency medications (e.g., midazolam) and monitoring. EMP’s and reports to support funding.
Holistic clinical nursing assessments, care-planning for high-intensity needs, clinical reporting and recommendations etc.
Registered nurses can provide custom training to informal carers or family members on specific health-related supports, such as administering medication or using medical equipment.
An RN for on-site short term or long term civil contracts (government and private enterprise).
Our nurses are uniquely placed to effect change, with decades of experience working within disability and mental health settings, including acute inpatient care, neuropsychiatric assessment units, community/ primary health nursing, and institutional disability care. We possess a very unique set of skills not found elsewhere.
Funding is based on the individual participant’s needs, outlined in their NDIS plan. For services to be funded, they must meet the criteria of being related to the participant’s disability, not the responsibility of other mainstream health services, and deemed reasonable and necessary.
