We offer consistent diabetes support in Newcastle and Lake Macquarie, to help individuals live safely and confidently within the community. Whether someone has just been diagnosed or has lived with diabetes for years, our experienced nurses provide education, monitoring, and daily support tailored to each person’s needs and routine. We specialise in assisting those who experience complex health issues or challenging and severe behavioural disturbances to effectively manage their Diabetes at home. 
Who We Support
We support people with all types of diabetes, including Type 1, Type 2, and gestational diabetes, and are skilled in working with participants who have additional physical, intellectual, or mental health needs.
 
Our services are ideal for people who:
 
 – Require support with blood sugar monitoring
 – Need help with insulin administration
 – Are at risk of complications from poor diabetes control

Our Diabetes Support Services
RN Plus nurses can assist with:
 
 – Blood glucose monitoring and record-keeping
 – Insulin injections (including pens and syringes)
 – Sliding scale insulin management
 – Medication prompting and administration
 – Monitoring for symptoms of hypoglycaemia or high blood sugar
 – Foot checks and skin care to monitor and reduce complication risks
 – Care coordination with GPs, endocrinologists, and diabetes educators
 – Dietary guidance and meal routines
 
Whether support is needed daily, weekly, or as part of a broader care plan, we’ll tailor services to fit your routine and goals.
